To Apply for Long-Term Care

If you are a caregiver with a loved one who can no longer live safely or independently at home due to their complex health care needs such as progression of a chronic illness, disability, or mental frailty, contact Fraser Health and ask for an assessment. The Abbotsford office number is 604-556-5000. A Case Manager will arrange to visit, and then make an assessment based on physical, mental, and social factors. If your loved one is currently in hospital, the team there will assist you with the process.

Maplewood Care Society receives primary funding through the Fraser Health Authority and all admissions are arranged through Fraser Health. Preferred Care Home choice is accommodated whenever possible, but urgent admissions usually go to the first available bed in the region. If residents wish to move after admission, they may put their name on a transfer waitlist for another Care Home.
